Get Assessed for a Clearer Path to Care

Every situation is unique. An assessment is the first step toward matching you with the right supports. Our team will guide you through the process so you can make informed decisions with confidence.

  • Our Information & Access Specialists (I&A Specialists) take a person-centered approach with all callers, working with you to determine strengths, needs, preferences, and what is important to you and/or the person for whom you are calling.
  • They determine with you the priority of the needs through conversation, then identify and discuss appropriate resources, allowing you to make an informed choice.
  • They may ask some demographic and financial questions that will assist them in offering those resources that may be available.

Get Services That Support You Wherever You Call Home

After your assessment, our team helps you take the next steps by connecting you with services that can support you in the place you call home. You’ll have the guidance of our I&A Specialists at every step so you can feel confident moving forward. 

  • After the initial intake with an I&A Specialist is complete, they will assist in making or scheduling an intake for internal programs. This can include programs with support for individuals, caregivers, and care recipients in their home or location of their choice, as well as financial assistance.
  • Depending on the program selected, this might include a more in-depth phone assessment and/or review of financial statements.
  • I&A Specialists can also provide information on external programs, including calling with you to complete a “warm hand-off” to assist you or your loved one in getting connected to other community programs.

For those in need of legal assistance or ombudsman services, or those who suspect abuse, neglect, or exploitation of an older adult.

Provides on-the-job training, employment assistance, and computer training to older adults.

Caregiver Services

Supporting caregivers with training and resources on their caregiving journey.

Dementia Support

Provides information and support to those living with dementia and their care partners.

Home-Based Care

Assists older adults and adults with disabilities with options to remain living in their own homes and the community.

Medicare Counseling (SHIP)

Highly-trained volunteer counselors are available to provide free, unbiased, high-quality, and accessible health benefit information.

Transition Services

Provides assistance to transitioning to a new home or home after a hospital stay.

Volunteer Services

Connecting volunteers through friendly reassurance calls, the Good Neighbor Network, Senior Companions, Foster Grandparents, and more!
